Ron; I am way too busy with debugging today to respond in full. I went to the FCC web site and entered "QAM tuning" into the search box. The first hit was this NPRM pdf With this text "In this area, we seek comment on whether television receivers will be digital cable (QAM)-ready, or 1394 ready, and when such sets would be available to the public." That was 1998, FCC 98-153 in CS Docket 98-120. There are more recent cites. I've aggregated some of these on my web site, but I haven't had the time to abstract them. But, debugging doesn't last forever, I'm told.
