It appears that SDL 2.0 textures must be 32bpp. However, Odamex's
IWindowSurface instance can be 8bpp. This will require
ISDL20TextureSuraceManager to perform 8bpp to 32bpp conversion on the fly when
pushing the surface to the texture and ultimately to the screen.
