[nvda] revision 253 - improved document reloading and ordered list numbering in mozilla

Hi all,
This revision improves the logic for when to reload a document in mozilla
virtualBuffers. It now uses a combination of both gainFocus and stateChange
to work it out. Previously there was a chance that you could land focus on
the document and it would just keep saying it was busy and never end up
loading the document.
Also now mozilla virtualBuffers report proper numbering or other symbols for
ordered lists, rather than just saying 'bullet'.
