Yeah that is a known problem with Microsoft MSAA itself, Narrator which uses
MSAA also does the same thing. The problem in most cases comes from a
little picture icon that programmers like to place before the text in a menu
item. The way that screen readers like Jaws etc. get around this problem is
by using either display hooks or, API hooks.

Subject: [nvda] rev1022 and menu
I don't remember if I'm using rev1022, anyway this is the latest trunch
available.
I've a problem with menus:
if I am in a folder, and I make right click to bring up the context menu
for a file, NVDA reads only some items.
In particular, if I select "open with", NVDA tells 1 of 8, 2 of 8, and
speaks only the item "program" 6 of 8.
here is an example with a doc files:
this may changes from pc to pc because anyone has his applications
installed, but just to give an idea:
file test.doc. I make a right click:
Open 1 of 22.
print 2 of 22.
check test.doc 3 of 22 (this is avast).
5 of 22.
open whith submenu.
submenu.
winrar submenu.
send to submenu.
cut 15 of 22.
copy 16 of 22.
create link 18 of 22.
rename 19 of 22.
delete 20 of 22.
properties 22 of 22.
is it clear?
