Hello,
there is an application called regsvr32.exe within the windows\system32
folder. You have to give the path to IA2.dll as a commandline parameter
to this application in order to register it.
So first go to your NVDA directory, locate ia2.dll in the lib subfolder.
Then go to the command prompt and type
regsvr32.exe x:\path\to\your\nvda\lib\ia2.dll
where x is the drive letter of the device where you have your NVDA and
path\to\your\nvda is the path to the folder where nvda.exe or nvda.piw
in case you are runing from sources resides.
Hope that's it.

context menu for a file, NVDA reads only some items.
In particular, if I select "open with", NVDA tells 1 of 8, 2 of
