It won't autostart unless you use a U3 memory stick.

Windows will pop up a message offering a number of options when the
stick is inserted. If the autorun.inf file is correctly populated the
default option (i.e. the one activated by simply pressing return) can be
"Start N.V.D.A."

If you use the U3 mechanism you can have NVDA start up without any user
intervention, but you have to supply some extra configuration files as
well as the autorun.inf file.
