[nvda] Revision 316 - insert+b script, Mozilla infinit loop fix, leaving menu foreground change fix

Hi all,

Revision 316 has the following changes:
*Added a script to read the current foreground window and its descendants (useful for reading dialog boxes). This is bound to insert+b.
*Fixed a problem when sometimes leaving menus, the next foreground change would not be spoken (examples: choosing control pannel or shutdown from the start menu).
*No longer descend in to 'embed' objects in Mozilla Gecko virtualBuffers. it would cause infinit loops. Example website www.expedia.com/ (There is a flash applet or something there).
