explorer window, then release alt
7. Hit alt+tab again and release alt

I didn't ask this to suggest that it should be the way you navigate folder
items, nor did I ask it to waste your time. I asked it as a way of
debugging the situation. I am trying to understand what is happening here.
No one else can reproduce the issue, so I need to know specific
information about your situation.
If you are interested in debugging this, please do the following and
report:
1. Reproduce the issue; i.e. get your system to a state where NVDA refuses
to read list items.
2. Press NVDA+f12. Does NVDA say the time? This determines whether NVDA
has frozen.
3. Press NVDA+tab. Does NVDA read the current list item? This determines
whether NVDA can find the current focus at all.
4. Move to a different item.
5. Repeat step 3.
