Are you able to download it onto your desktop and then bring up Preview and 
open the picture from there by selecting it with the open command from your 
desktop.  
Also does it have .jpg or .tiff file type?  If it is not any of the other 
listed items in then it may be greyed out and you will not be able to open it 
with Preview.

I'm probably doing something wrong  but when I click on the  
attachment, go to "file" and click on "open with" I get a dialogue  
box with many applications, none of which is preview, which, as you  
say Andy, is already installed. There must be a simple move I've
