Nope, actually it's an InDesign file that once had a JPEG or an EPS attached to 
it, but somehow the attachment has been broken. Normally, I would have embedded 
it, so that image and text are merged in a single file, but this time I must 
just have attached it (as we used to do at the Free Press in Quark) and the 
original image file appears to have disappeared, breaking its link with the 
InDesign page file. That's why you can't get it into iPhoto as is; it's not a 
real photo image any more, just a low-res artefact InDesign is
improvising to take its place (the way it improvises replacement text when you 
try to open a page file that was originally formatted with a typeface you don't 
have).

What you could do is enlarge the PDF to full size and take a screen shot of it 
(Command-Shift-3 to capture the full screen, or Command-Shift-4 to select just 
the picture portion of it). The captured image will be able to be saved to 
iPhoto; you might have to reformat it to a JPEG, but I shouldn't think so).
