It could be that this is a file that was "updated" by Apple's updater. One 
of the flaws of Apple's software install in OS X 10.3 is that it'll place 
files in places where it _thinks_ the host application folders should be. 
Thus, if you move iTunes out of the Applications folder (e.g. to a 
sub-folder like I have) the updater will place updated iTunes file in a 
make-shift iTunes.app folder in /Applications/ when it can't find 
/Applications/iTunes.app/. This means that you'll still have your un-updated 
iTunes.app sitting somewhere else and a non-functional iTunes.app in the 
Applications folder which contains only updated iTunes files. You can fix 
this by manually moving the files from the "updated" iTunes.app container to 
the "unupdated" iTunes.app.

Why do I speak of updated files? Well, Unix (& OS X) doesn't use "resource" 
files anymore (OS 9 used resource forks to store such things as application 
graphics, code, menu bars, warnings, etc. in easily edited formats and data 
forks for, well, data (like text documents, pictures, etc.)). Such info is 
stored in files which are contained within a folder which OS X treats as an 
application. When an app gets updated most of the stuff is left alone (e.g. 
menus, graphics, etc.) and only a few things need changing. Thus, only those 
few files are changed.
