[mso] to make a complete form

I have created a form where i can make the list of the clients and i want to 
make a form where i can add the name, father name, age and other optional 
information about the clients with their photos, and after that when ever i 
someone want to see the list of clients i want them to see the photos with 
there ID.
  please tell me how can I do this.
