Hello David,
I am in Print Layout and see the background just fine in Word 2003, but when
I PDF it, no background. =(

Camille: Sorry if I get it all wrong by jumping in in the middle of the
thread. But aren't background effects limited to web pages? I mean, you have
to be in Web Layout view before the Background command appears on the Format
menu.
When I use PDFMaker to create a PDF of a web page, Word toggles to Print
Layout view and PDFs the underlying document.
I'm working towards saying that what you want may seem reasonable, but may
not be possible. If you print a web page that you're viewing in IE with a
background, do you expect to get the background too?
You may need to work from Page Layout view and create the page background by
inserting it like you would a watermark, in the Header/Footer.
