When doing just this a couple years ago we did it with Superscript and
subscript by writing a Macro and some experimenting a bit then it was
just a matter of one click and the word appeared?
Maybe you could do the same?

I am transcribing some old wills and have noticed the word "and" is always
spelled starting at the superscript area, and slants downward at 45 degrees.
