I'm not attempting to answer all parts of your question, but I just did =
a quick experiment with Word 2000. The built-in index mark function only =
suggests two levels of index (main entry and subentry). However, by =
editing the field code, I found I could create at least four levels. So =
you could create your multiple indexes that way. The field code would =
look like:

{ XE "COIN COLLECTING:Tennessee:Knoxville:O'Scanlon, David }
{ XE "COIN COLLECTING:Tennessee:Nashville:Purcell, Richard }
{ XE "FARMER:Kentucky:Christian County:Markswell, Julian }

etc.

Ooops!  Responding to my own post. In my example, I left page numbers =
off
the example for "Employment / Pastime Index." I meant for it to look =
like
this instead:
