I don't have Outlook at home so I will try to do this by memory:
With your contact file open, choose File-Export - there are various steps to
follow but in the end you end up with an Excel spreadsheet with all of your
contacts listed in rows and the various fields across the top, each a column
head.
Save and close that file.
Now go into Word and set up your document.
Using mailmerge, identify the source of your data (the spreadsheet you just
saved.)
Then line by line you will insert insert each mergefield (name, address,
phone, etc.) Before each merge field, type out the name of the field. So
the end result will be:
Name: John Doe
Address: 123 Main
Phone: 123-1234
E-mail: whatever@xxxxxxx
etc.
After you've set up your document, you then "Preview results" to make sure
it looks the way you want it. Then, "complete the merge" and choose e-mail
option.
Then watch it go!
I've modified and attached the two files I used to produce my merge to
e-mail. There is a spreadsheet with 10 records and the mail merge
letter. (the mailmerge I did resulted in 700 personalized e-mails.)
While my spreadsheet has fewer fields than yours will, you will hopefully
get the idea.
