[mso] Microsoft Word Merging directory fields

When merging a directory of names and addresses from different countries I want
to include the name of the country for the first instance only and then ignore
the country name until it changes - as a statement I want something like
IF COUNTRYFIELD EQUALS LAST COUNTRYFIELD LEAVE BLANK ELSE COUNTRYFIELD
Used to be able to do this on the Mac years ago when Word wasn't so 'helpful'
and can do it in Quark using QuarkXPress but cannot find a way in Word 2003.
Can anyone help please?
