  In solving your problem you can use transpose (pastespecial function) and do 
a little macro so you wont do the process for all set of records.
  
Also, you can create a formula that links to your data without neccesarily 
going thru all records.
   
  assuming that your data starts in cell B5, im refering to the actual data and 
not the data title (ie Name, Address etc)
   
  Enter the following 
            E         F     G          H               I                       
J                     K                     L                      M         
       5     xxx=    B     5        =E5&F5&G5   =E5&F5&G5+1   =E5&F5&G5+2   
=E5&F5&G5+3   =E5&F5&G5+4  =E5&F5&G5+5
       6     xxx=    B     11      =E5&F5&G5   =E5&F5&G5+1   =E5&F5&G5+2   
=E5&F5&G5+3   =E5&F5&G5+4  =E5&F5&G5+5
       7     xxx=    B     17      =E5&F5&G5   =E5&F5&G5+1   =E5&F5&G5+2   
=E5&F5&G5+3   =E5&F5&G5+4  =E5&F5&G5+5
       8
       9
   
  Select Cells "E5" to "M7" and Copy/Drag the formula up to the how many rows 
that will accomodate your data.
  Afterwards select columns H to M and copy paste special (value).
  Then, ctrl+F put the xxx in the "find what" field, press alt+R then press 
alt+A
