Oh, I can use a smaller font but where the form asks for an Agency
name, address, phone and fax numbers - I have three (3) two separate
agencies plus my private practice. So I have three different agency
names, three different addresses,... You get the picture.

I just wanted to add copies of lines on the form but because it is
locked, the only place I can enter information are in the fields.

I don't know where the form was originated so I can't address the
issue to anyone particularly. The bishop's office doesn't know
anything about it.
