[mso] Controlling Headers & Colorize table of contents?

Hello everyone.
I have a document whose table of contents is laid out like a table. For
ease of readability, I tried to put in a header describing each column,
so it would be more readable. I did that, but there is a problem. Word
wants to repeat this completely throughout the document. Is there a way
to limit the headers to just the Table of Contents pages? I am aware of
the "Same as previous" button, but it seems to stay grayed out. When I
check Internet sites about how to control this, they seem to all suggest
making use of this "Same as previous" button, however, it doesn't seem
to want to go "hot" for me. How is that done?
And, I would love to make the table of contents highlighted every third
row. In Excel, I could do it, but how is that done in Microsoft Word? I
am using Word 2000.
