Perhaps I can help here.
There are two ways of doing this, the Wizard way and the Full Works way
First of all, create your form from the original table without any lookups
using the Form Wizard. At this point you should be able to add and edit
data.
Now, you have a choice.
The Wizard Way
==========
In Design view, select the field you wish to convert to a lookup field,
Select Properties, click on the Data Tab and write down the Control Source
Name.
Delete the control.
On the Form Design Toolbar, Ensure the Control Wizards button is selected.
Drag a Combo Box or a List box from the toolbar to the right place on your
form and the Combo Box Wizard will open.. This will lead you though the
steps required to select a field from another table. When you get to 'Store
the selected value in this field.' select the Control Source Name you
recorded earlier.
The Full Works Way
============
Go to Design View, select the control you wish to convert to a lookup field
and right click on it. From the Menu, select Change To and select Combo Box
or List Box.
Right click on your new control and select Properties.
On the Data tab, select Row Source and click on the button marked with
three dots at the end of the row.
This will open up a Query Builder form with a SHOW TABLE box in the
foreground allowing you to choose from where you want to get the data.
Select the table from where you want to get the data, and click ADD and then
click on CLOSE.
The table will now appear in the top section of the Query Builder.
Drag the field(s) you want to display from the selected table into the field
section of the query grid.
When you have finished, close the Query Builder form.
You will then get a message "Do you want to save the changes made to the SQL
Statement and update the property?"
Click on YES.
Save your form.
Your Combo or list box will now show the new data
Easy when you know how..
