[mso] Access 2003 Switchboard form

Hello All,
 
I'm looking for some instructions on how to create a Switchboard form in Access 
2003. What I have is a database that I need to create  switchboard for that 
gives the basics of the database along with my name and contact information. 
Any suggestions or a link that can show me instructions on creating a 
switchboard form for my database?
