Turntables were at nearly every railroad terminal in the USA though the GM&N 
at Jackson, TN had a wye and just a three stall engine house.  Both the M&O 
(later GM&O) and IC had turntables at Jackson and the IC had another one at 
Fulton, KY about 80 miles north of Jackson.

As to the engineer more or less sitting beside the firebox, this was common 
practice over here and I am sure it was pretty warm.  Farther North, and in 
the Western mountains, cabs were completely enclosed in the winter 
especially.  Some locos had a heavy curtain that closed off the sides in bad 
weather.

I think the photo of the fireman seemingly tossing coal in at nearly deck 
level is that the photo was made while he was in the process of tossing in 
the coal higher up or else removing his shovel for another scoop.  Most 
firedoors were about 2 to 3 feet above the cab footplate.
