Turning Top Sites on or off makes no difference to the accessibility of Safari 4. The Top Sites window...or tab...is not working with VO, so if you stay planted in that window it might appear that Safari isn't working, but I've left it on for experimentation's sake. There are a number of great access improvements. Mouseover text is read properly, same page or anchored links are now working, and the responsiveness is dramatically better than Safari 3.
