I have no problems with the insertion point of the iphone except in forms on the web. when in native apps like the contacts list, mss and mail, it behaves the same as the insertion point on the mac. In other words, if you set your rotor to characters, flicking up and down will flick you through the characters in an edit field such that flicking up will put you between the character you just heard and the one above it and flicking down will put you between the character you just heard and the one below it. this means that if you are flicking down and you hear a character, pressing delete will delete that character and typing a character will insert a character below it. Flicking up and pressing delete will delete the character above the one you heard and inserting after flicking up will insert above what you just heard.
