Hmm, I'm on Sprint. Interesting point. At first, I thought it was a 4 compared to 4S thing because, my iPhone 4 was on At&T and my 4S is on Sprint. I didn't think the carrier could be the reason of lower VO volume while on a call on speaker. But now, this just might be the case.
