Sometimes, holding the option key down too long when using the vo combination will cause this behavior; this isn't a bug but is how it's supposed to work except that it can be done unintentionally when using voiceover. Other times I think I haven't held down option too long but it still happens. Also if you just close iTunes and re-open it, this usually doesn't happen again.
