In Ontario, we don't pay for a doctor's visit. We don't pay for a catscan. The amoxycillin, I can't say, because my insurance at the school here pays it all. It also pays for eyeglasses and dental procedures. The government publishes a fee schedule, of course, for all medical procedures, but I'd have to research that. Don't have it to hand. My mother has had three major brain surgeries to remove a tumour which keeps coming back. The first was in the US in the late 70s and, even with her blue cross, she paid on this for years. The other two were both done in Toronto after she immigrated to Canada and she never paid a penny. Our taxes are higher, but I don't complain.
