My approach to this is simliar to David's with some differences in the details. 
 Judging from the resulting photos it seems that both David's and my approaches 
to using the 400 f/6.8 with the DMR work well.

Instead of adding a grip to the shoulder stock, I added a QR plate where the 
flat bar of the shoulder stock has a 1/4" threaded hole.  My monopod (Bogen 
3218) is topped with a tilt head (Bogen 3232) and a QR clamp (RRS I believe) 
and I keep my shutter trigger hand on the camera.
