I have an individual policy with Anthem BCBS, called
Community Choice.  1 person coverage with a $500.
deductible is ,$200./mo at the standard rate.  It also
takes into consideration preexisting conditions.  To
have a blind application run through to assure
approval and avoid being blacklisted by rejection I
went through an agent.
