for the client in the use of basic community resources; and monitor overall 
service delivery.

Requirements_Qualif: Bachelor's degree and two years experience in Human 
Services or related field.  Excellent communication, documentation, time 
management, coordination and collaboration skills; ability to mobilize 
community resources and to coordinate 
in-home and transitional services; experience in crisis intervention; basic 
knowlege of developing service plans. Driver's license and car for frequent
