HomeLink Transitional Support Specialist

Full time position to provide intensive support services to SMD clients =
while assisting with placement into housing and work as part of a team =
to provide case managers and clients with a housing and residential =
network.

Bachelor's degree in human services field and two years experience or =
equivalent combination of education and experience in a human services =
agency; total combination of six years education and experience in a =
community-based mental health program and experience working intensively =
with SMD population required. Experience working with homeless =
individuals a plus. Excellent time management, communication and =
documentation skills; ability to be flexible with work schedule (to =
include some evening and weekend hours); and valid driver's license, =
vehicle and insurable driving record are required.
