[jsfg_cinti] Contracting Questions

To All :

My name is Jason White and I have been a JSFG member since August.  I recently 
received a call about doing some contract work for a company, and have a few 
questions:

1.  Does anyone have a "blank" form or contract that they can send to me 
electronically?  I have never contracted before, and would like some sort of a 
legal contract.

2.  What do I need to do about unemployment?  Obviously, I fill out my card 
with the correct income level, but what happens when the contract ends?  How do 
I refile?  What risks are there to refiling?  What if I am working for myself, 
and not a contract house?  I do not have a federal or state business account, 
so I am not too sure in this realm of things.

Any help woudl greatly be appreciated.
