      Description:   
      Non-profit rehab hospital (LTAC). The Care Coordination Leader has 
24-hour responsibility and accountability for coordinating and managing the 
delivery of patient/resident care on an assigned clinical unit from admission 
through discharge. The CCL and their designated team of staff members will be 
assigned to these patients. The responsibilities include: planning, 
coordinating and evaluating patient care outcomes in consultative and 
collaboration with physicians and other health care team members. Appropriately 
allocates and manages available human, financial and material resources to 
promote the delivery of quality-focused and cost-effective patient/resident 
centered care. Has overall responsibility for patient/resident chart 
documentation, developing staff and facilitating teamwork, evaluating and 
enhancing nursing care quality and maintaining the operational aspects of the 
unit. Provides focus and direction to unit quality improvement initiatives and 
implements appropriate practice/system changes based upon findings from 
monitoring and evaluation activities. Salary: $45K-$49.7  
      Requirements:   
      Must be a graduate of an NLN accredited school of nursing--Bachelor's 
degree is preferred. Must have current license to practice as an RN in Ohio. 
Must have BCLS and ACLS specific to rehabilitation, medical/surgical, 
orthopedic or gerontology nursing. Must also have 2+ years experience in LTAC
