With the example of s t period, if it is a phrase which I will run across a 
lot, I put the phrase into the dictionary. For example, st. louis, all in 
lower case letters, is pronounced saint louis. I might do the same thing for 
someone like, maybe, dr. livingston, I presume, putting in the phrase dr. 
livingston and modifying it to say doctor livingston.
