I've got the same problem, but I've found that if you tab once, then
shift-tab back, then it will read the mis-spelled word. The odd thing is
that it's not consistent. Some times it will read the spell checker just
fine, and other times it won't.

Hi Everybody,
Well, my update wouldn't go in via the automatic feature but I did download
the full installer only to find out that spell checker in outlook express is
still broken for me under windows xp home edition with an nvidia video card.
For me it doesn't announce the misspelled word at all no matter what I do.
Has anyone else encountered this difficulty? I reported it during the beta
cycle but sadly at least here on my system it seems to persist (frown).

If it told you that the latest update was already installed, then there's something wrong with the update process. I would suggest you go to the link Debbie gave and download the update. It's a full install and will install overtop of your current JAWS71 instalation, maintaining all your settings. I've been working with this update yesterday and am glad to report that some of the problems I was having with 7.1 are resolved nicely.
