I already tried this. If I could do that, I'd have more than a one-octave
reach on a piano keyboard. But alas, I can't manage this. And I don't have
particularly small hands. This is definitely not one of the most ergonomic
key combinations around, I'm afraid.

You can use the thumb and pink of the right hand for insert and alt and S as
