I forwarded your message to the people at FS that would know about the DEC
Access and Jaws settings, and this is what they said:

DECTalk Access32, like DECTalk Express, is not recognized by the dictionary
manager if you choose a language in which you want to make the addition to the
dictionary. For example, when you add an entry, the default language setting is
"all languages" and when you then tab, you land on a control that basically
says, "Speak replacement word using Language: Default." However, if you change
that language setting to something else other than "all languages," then
the list of synthesizers that support the chosen language appears. For example
if you choose English, all the installed English synthesizers appear and
you can then use that specific synthesizer, but since DECTalk isn't supported,
neither Access32 nor Express, it will not be displayed. I thought we had
help text discussing this in detail.
So, in short, if Gary wants to make an entry stick for DECTalk Access32, he
will have to set the entry for "all languages." If he wants to make it
stick for just Eloquence or one of the installed RealSpeak voices, when he
chooses a language from the combo box, the supported synthesizers appear in
the synthesizer combo box.
