Get Jaws window on screen with Insert + J
Go to menu bar using the alt key.
Open options menu with down arrow.
Tab a few times and you'll get a message: Start Jaws automatically?

Tab on down to OK and press Enter or Space.

As for a hot key, my method is to ensure that Jaws icon on the desk top 
is the only one starting with J, then, if I've stopped it for any 
reason, I do Windows key + M to minimise all and put focus on the 
desktop, push J to select the Jaws icon, then Enter to start it.
(There may be simpler ways, but this works for me.
