Martin,
If I were in your position the two screen readers I would investigate are 
system access.

I think this £220 for system access with upgrades free for the first year 
and then seventy pounds after that.

And the Mac screen reader voice over, of which there will be a new version 
in the new year.  Try having a look at the Mac visionaries site.  Sorry not 
sure of the address.
