I must admit that I have this sort of thing happen with USB flash drives
on occasion.

I now have an Obey file with the line

  RMReinit scsifs

in my Library, stuck on the pinboard, and I have developed the habit of
always running this before acessing any newly-inserted USB device.

After a number of weeks, I can state that I have never had this problem
once, since starting this procedure.
