Also, check that the fan on the card is still working: I had a similar
problem which resolved as part of the memory on the card had burned out due
to the fan having stopped - it is so quiet that one does not notice if it
fails.
Like you, I found that sometimes the screen was fine,
Having replaced the card with the FX5200, I have had no more problems.
