Have you tried plugging it into a Windows machine,  I seem to remember
that this may have helped when this happened to me.

Have you tried !ROFS ?  (not available when this happened to me)

When this happened to me I had to reformat the pen drive (it came with a
Windows management program), I then named the empty drive on the Iyonix
and swore never to rename the drive again.  This seems to have worked OK
ever since.
