Has the new disc been set as bootable? Do a:-

  *OPT 4 2
  
Check that its also trying to boot off the right disc by doing a:-

  *Configure FileSystem ADFS
  *Configure Drive 4

That shouldn't be necessary, either something is unplugging it each time, or
the CMOS memory is corrupt.

Are you using standard devices directly connected, or wireless or connected
via a hub or KVM? Reducing to the simplest combination while testing is
advisable.

The small card holds the high lines of the 64bit PCI bus which was needed to
stop them drifting on early systems, But it cant be fitted if you are using
both of the 64bit slots, as you are with the full load of 4 PCI cards. It
wont be needed if any of the cards you are using is 64bit, but probably they
aren't.

I think the only thing thats non fatal and would allow an escape to continue
is waiting for a storage device to become available.
