Web publishing and images.

Hello,
I am publishing an internal website and for some reason certain images do not
make it through when you hit a page. The images are published fine on the
back-end server but the are left blank when you look at the published
version. Is there anything obvious that I should be looking for. Once in a
while they publish ok. I just can't determine what is causing this. Could
there be cache polution and if so how do I fix that?
