Some more detail would be good.
 
the main website is hosted elsewhere, does that mean that the webpage
with the picture is the only part behind isa?  If so, then if he uses
the fqdn, it will just go to the website, not the isa box.
 
He would need to set up a web publishing rule pointing to the internal
page for the ip addy pointer to work, and set the ttl to expire
immediately.

My buddy who is working with ISA has come up on another troublesome
problem. 

He needs to publish a single file (.JPG) through ISA. His website
(hosted elsewhere) has a page that displays this .JPG file which changes
every few minutes.  He hasn't been able to get it published thru ISA
even tho he says he followed the instructions for web publishing to the
letter.

Today it occurred to him; the pointer to that .JPG file on the webpage
uses the external IP address of the ISA server and not the FQDN of the
ISA server.  Could that make a difference in the success of publishing
the file?
