I am trying to answer the oringinal question.
MMAGNO: I meet the same problem just by using OWA itself. Now I fixed
it. Does your Exchange server host more than one SMTP Virtual Server? If
that is true, there must be more than one Recipient Policies listed.
Lookup the following:

Mailboxes Must Have SMTP E-mail Addresses That Conform to the Default
Recipient Policy
An upgraded or moved user may not have an SMTP e-mail address that
matches the Default Recipient Policy. In order for a user to access
their mailbox using Outlook Web Access or Web Folders, an SMTP e-mail
address matching the Default Recipient Policy must be added to the user
object. To add an SMTP e-mail address to the user object, start the
Active Directory Users and Computers Microsoft Management Console (MMC)
snap-in, open the relevant user's properties, select the E-mail
Addresses tab, select the SMTP address, and then click the Edit button.
Then verify that the SMTP address matches the SMTP address in the
Default Recipient Policy.

See if with the administrator user you can access the OWA, and see the
virtual directory exchange settings

Are you sure it is authenticating properly from someone INSIDE the
firewall? Sounds more likely to be an OWA issue than a firewall issue.

I Have a OWA 2000 behind a Isa Server that fails to
authenticate. It displays the "please type your username and password"
box three times, and after that fails with the message "Error: Access is
Denied."
