The destination set has to include any host that you expect to allow.
If you want external folks to use "http://host.domain.tld";, then you

It returned "http://mail1.tyeecap.com/exchange - 12202 Default rule - "

What makes my head hurt is that I can get through on the 172.16.0.x
network but not from outside the Symantec 360. The error message is
definitely the ISA.

Does the Destination set have to use "mail1.tyeecap.com" or can it use
172.16.0.2 for the Name/IP Range?

403 + 12202 = rule-based denial.
Either you have no rule allowing this traffic or a specific rule
blocking it.
Add "Rule#1" and "Rule#2" to your web proxy logs and retry the
connection.
