Qmail is a pain to config :(
I will stick to postfix / Exim
What I see problem with qmail is see how many packages in qmail home page.
Sendmail comes with most of the packages. I configure sendmail,
if required call some thing like mailscanner and install anti virus and
tell mailscanner about virus scanner. If required do auth mail or any
other addon are simple issues in sendmail.
Qmail security is achived because of its less code base ( one of the
reason). But we need more to do present email system. If you afford
one mail server doing qmail barebones and internal sendmail doing
all checks may be a better option.
