I can't speak for anyone else, but I make them a pin.
A multidrill one. It works out well. I tried to not add
an internal pad because the holes would attach to ground
but if you have drills and no pads the router does not see
the drills and routes right through them. That I discovered
by accident, thankfully BEFORE I sent the design out.
They need all of the pin stuff so it just made the most
sense to me to do it this way.
